"Shure Sleeves" I read customer reviews and brought the Shure triple flange sleeves. I was shocked and disappointed that the ear sleeves doesn't block out the ambient noise; poor noise isolation. In addition, they don't fit comfortable in my ear. I believe the best sleeves for noise isolation is the Shure gray rubber sleeves. Learn and live.
"3-way flanges" Worth it. I wish they shipped with the E3c's. I trimmed off the end for my ears. My wife left all three flanges for herself.
"These actually fit!!" I had a pair of Harmon Kardon headphones with the triple flange sleeve. I wanted to get a little bit of a better pair so I got the Shure E4C's. If you have an odd sized ear the triple flanges are the best option if the regular buds fall out of your ear. At times they are not the most comfortable, but it's just a small sacrifice to pay for their performance.
"Great quality sound. Rubber quality issues." First of all, please remember that different people have different ears that fit different sizes and shapes of plugs, so please do not judge the product based on the very subjective matter of ear compatibility.
The sleeves themselves provide very good sound isolation. Even with the music off, they can reduce the deafening rattle of the subway to a lulling hum. Put the music on, and you're cut off from the world! That can be good and bad. When you do want to be cut off, it's great; but I wouldn't recommend walking in traffic with these things in your ears - you may get run over. Also, if people are talking to you, don't expect to hear them.
I absolutely love these things except for one detail. They don't last long. They will come clean and white, but the rubber will deteriorate with time and turn hard and yellow whether or not you've ever put them in your ears. They come three pairs at a time; one I used, the other two I kept spare, and within just a few months all pairs were an ugly yellow. The ones I hadn't used also turned hard and inflexible. The one I used got too soft and slimy, and even after I scrubbed them with soap inside and out, the plugs just wouldn't stay on the earphone tips anymore, but slide off and fall.
For the sound quality and isolation, these are definitely worth getting. I'd just recommend buying one pair at a time, or if not (like these come three pairs together) then changing them often, before they have hit their expiration date.
p.s. I use them on my Westone earphones, which they are compatible with.
"Work Great for Etymotic ER-4 Too!" I've got Etymotic ER-4 headphones -- which sound great, by the way -- and these sleeves fit perfectly. Even better, they give just a little more space between your eardrum and the speaker. This allows a slight expansion of the stereo field, which I'm enjoying immensely.

"Weird, but functional", They isolate well, and do indeed narrow the soundstage of my se110s. Unfortunately, they look very strange and reach into parts of my ear that I was not aware existed. Not exactly uncomfortable, just unusual. I'm going to stick to the foam tips for now, but when they wear out, these will do.
"These things HURT your ears", I hated these things.
1. They hurt your ears, leaving them sore after only 30 minutes of listening time.
2. They are frustatingly difficult to insert into your ears. I suggest either a lubricant or some romance to get the ears "in the mood" first. Or you can just skip this purchase altogether.
3. They are not really necessary since the other "buds" do just fine for most activities.
"Very Comfortable", They are difficult to get into your ear, but once there they will not fall out. They are perfect at blocking all sound and I found them to be very comfortable.
Only complaint, they start to turn yellow from ear wax, so make sure to wash them every couple of weeks or once a month.
"Simply amazing", Ordered these sleeves to use with Westone UM1 headphones and Sansa Fuze MP3 player. Before these arrived I used to use Comply tips, which came with Westones. They were never satisfactory for me: not easy to put in and required some serious EQ to sound right, which obviously had an effect on the sound quality
On the contrary these triple flange sleeves are simply amazing: very easy to put, stay in place and the sound is dead on with Fuze + UM1s. No EQing is needed any longer and this portable combo sounds almost as my reference desktop setup: Philips Aurilium + Audiotecnica ATH ES7
And one more thing - I did bi flange mod to them, which did not affect the sound, but is much more comfortable
"Surprisingly bad - until mod", I just tried them with my E3c's and was surprised by how bad the sound was. Lost all bass -- like listening to my music in a tin can! However, they did stay in my ear better, but who cares?!
--UPDATE-- I've added 2 stars after taking some scissors and cutting off the top flange and shorting the stem at the other end. Seems to fit better and I got some of the bass back.

Read this reviews before You buy...
"Excellent Enhancement to User Experience", I fly and ride motorcyles long distances. In both cases I need headphones that are compact, and truly block noise. The addition of these flange sleeves make the Sure headphones perfect in this regard.
"These buds are great", I purchased the SE210 and over a few weeks, I tried all the tips and all the sizes. I kept having problems getting a good seal, and the tips would work lose as I hit the gym. Then I tried these tiple flange.
At first, they felt funny and when I pushed them in, the flanges would would sort of flip over and essentially block all sound. I was getting frustrated thinking the SE210 was not as good a bud as the E2C. When I first looked in the mirrow, they seemed to stick far out of the ear.
Then I read a review that helped me.
Since the flange was sticking out of the ear so far, I clipped off the third flange. That helped a lot. Then I read another article about getting each tip wet before inserting. (a little saliva will work fine) I next wet each tip before inserting, and each bud slid right in. No need to pull on the ear or any other odd trick. Once you get the flange bud in and drape wire behind the ear, the seal is great and the sound is excellent.
I just returned from a long flight on a small jet carrier and I had the buds in from take off to landing. The engine roar was totally blocked and I had no problems getting excellent bass from jazz or rock music.
I think users that state they have a problem just don't take the time to properly insert or prepare these tips. They might create a little soreness in the ear, but I cannot imagine a better bud or tip. I just wish these fit my E2C.
If you have any Shure buds these tips will fit, I think these flange tips will end up as your preference. They don't deteriorate as the foam ones do. They are softer than the silicon tips, and they won't fall out of the ear as the rubber tips tend to do when you perspire.
"Shure, there are better tips out there...", These are good for working out. I do not think they sound as good as the grey tips. If you want tips that will stay in your ear (they hurt my ears after prolonged wearing) and isolate outside noise then buy these. Otherwise, get the grey silicone tips or shure foam replacement tips.
"Good option for E3c.", The small size fits my ear perfectly. I wanted a second set for the E3c earphones that I purchased.
"Very good, but", I too, find thy are much more comfortable if modded. I use a razor blade I cut off about half of the stem from one end and the smallest flange from the other.
